Doubts in Completing Form MN1

Hi All,
I am having ILR from last 10 months now, I want to apply For Registration of my daughter who is currently in India but she was born in UK.
However, I have some queries.
1) I have attached these papers along with this Application form.
a) Child's birth certificate - Original (Issued from UK)
b) Her Indian Passport Issued in UK - Original
c) Father's ILR approval letter Copy and Bio metric Residence Permit Card's Copy
Are these enough or Do I need something else also ?
2) In Section 3.13 and 3.15, We need to fill about the parent who is an EEA National and claiming to have permanent residence since 30 April 2006. Do I need to full my details here for the last 5 years ?
3) Good Character Requirement- It is mentioned that this applies to the children if they are aged 10 and above. My daughter is only 2 years - Can I skip that part completely and fill only 4.11-4.17 ?
4) The fees is 673 Pounds but it is nowhere mentioned to enter the card/cheque details. Are they going to charge it later ?
Or Do I need to attach this also
