Naive questions which I shall still ask to avoid confusion...

Section 1.6 doesn't explicitly ask for first name. But, do I need to mention the first name here?

Wife's maiden name should be mentioned in section 1.7 or section 1.8?
Do I just mention the last name before marriage or both first name and last name? (First name has not changed after marriage)

I put my first and middle name in Section 1.6.

I would put my full maiden name (first and last name) in Section 1.7 if there is enough room. If there isn't enough room, put it on page 15 (additional info) and reference page 15 in Section 1.7.
