I am filling form AN. I am confused on section 1.5, 1.6, 1.7 and 1.8. I filled the following sections with these values.

Last Name

1.6
----
First Name

1.7
----
Full Name

I do not know whether I filled it right.

Any help will be highly appreciated.

Many people will leave Q1.7 blank. After all the question does say "if different".
