Form MN1 Section 1.1

I am an indian applying for Citizenship next month. I have child who was born in india. She is staying with me. She has also obtained ILR along with me lat year. Now I am applying citizenship next month . I wish to know what to what to fill in section 1.1 of form MN1?
I read chapter2 it is ver confusing.
