Hi,

UK PASSPORT SECTION 4 queries: -

1. Mother’s or Parent 1’s full name (surname first) - I think Mother name is sufficient please confirm.
2. Date of Birth - I do not have this information. Can I leave it blank?
3. Date of marriage - I do not have this information. Can I leave it blank?

Please let me know if my understanding is correct for these questions.

It depends on what basis you are applying the passport.
If you are applying on the basis of naturalization then 2 and 3 blank are fine. You will still need full name of your mother although I don't think it will be an issue.
