Northern Ireland - police form DAT, need help!

Has anyone recently applied from Northern Ireland and gone through the DAT1 form thing?
I have lived there 5 years ago, and now live in England, byt UKBA requested that I submit this form to PSNI ASAP. Unfortunately, the download link they gave me, www.psni.police.uk/DAT.pdf is not working right now (looks like the whole server is not responding to me). If anyone has the resonably recent PDF, could you let me know and I'll send my email address, so you could - please, please! - email me the PDF?
