how to write the name in 1.5 , only the surname or complete name if full name in which order????

For 1.5 it clearly states SURNAME - that is what you write
For 1.6 it is for the other names, i.e. first name and second name if there is one.
Surname and name(s) need to be exactly as it appears on birth certificate and passport.
