Dear Gents/Ladies:
Please advise on the following questions if you know

Applicant has resident in UK more than 8 years and obtained ILR through SET(m)as unmarried partner to UK citizen. now 12 months period has completed.

Question 1: Is the applicant qualified to apply naturalization?
Question 2: which form to fill in?

You can apply if you have been UK resident for the last 5 years and had ILR for one year.
