Form AN 3.5 empty for Self Employed, correct?

Hi
In Form AN field 3.5 must be empty if I'm self-employed, I don't have these numbers. I must send them Self Employed Statement instead. Correct?

Could anyone confirm please?
I don't use PAYE and P60 as I am Self Employed, so I need to send Statement of Account from HMRC to show that taxes been paid. I don't have tax office or tax office reference number, so I can leave this 3.5 field blank and send my Statement. Correct?

Yes, Thank you very much for your help!
I sent most recent Self Asseseement Statement from HMRC as they require.
I just became slightly worried that I left this 3.5 field empty, I think I don't have these tax office thing, it's for PAYE system. So I wanted to confirm that I did everything right, sending latest Statement and leaving this 3.5 field empty. So did I do it right?
