NATIONALITY REGISTRATION,APPLICATION MN1 UNDER 1(3)

Hello!
Could someone help me with this?
I am Portuguese and have been living in England for 5ys and 4months.
I hold a Permanent Residence Card, issued on 09/12/2014.
I am aware that my children are now entitled to be registered as a British Citizen.
My queries are;
On Application MN1 UNDER 1(3) do I need do fill the sections 3.13 and 3.15 or live in blank and just send to them my PR card as proof that I am already settled?
Can I use my PR card number as Home Office reference number?
