EU citizen filling AN form for naturalisation

Hi, I'm EU citizen living in UK since 2004. I have PR issued on 28 August 2014 and preparing for submitting an application for BC.
I need your advice on filling the AN form:
1.1 any previous reference from UKBA or HO - should I put here my PR application reference?
1.3 they ask about date ILR obtained - should I put date of my PR?
1.22 Life in the UK - I passed Life in the UK test in April 2012 - is it still valid?
1.22 Language requirements: I have PhD from the University of Manchester, UK - do I need to send it for validation to UK NARIC?
1.53 Employment history for last 10 years - do I have to fill it?
2.2 Absence from UK in last 5 years - do I need to fill it?
2.4 Please state on which basis you were in UK for last six years - Do I need to fill all six years as I have PR?
I would be very grateful for all comments and advice how to fill this form correctly.

1.53 Employment history for last 10 years - do I have to fill it? Yes

2.4 Please state on which basis you were in UK for last six years - Do I need to fill all six years as I have PR? Yes, I think you do need to complete this question
I would be very grateful for all comments and advice how to fill this form correctly.

Thanks a lot for all previous answers.
There is one more major area I’m not sure about.
I received permanent residence (PR) on 28 August 2014 on the basis of being EU citizen and exercising treaty rights between 2004 and 2009.
How should I fill in the AN form? Should I filled it for 2004-2010 period when I was actually exercising treaty rights or last 6 years that is what they request in the form AN that is 2009 -2015?
For example section 2.4 of MN form says: Please state on what basis you were in the United Kingdom for the last six years?
Can you please advise? Many thanks.

last 6 years that is what they request in the form AN that is 2009 -2015?

Thanks all for help and advice. My Citizenship form is ready now.
Currently, I am preparing documents to be attached to the application.
As a EU citizen with Permanent Residence status for over a year I should attach:
Proof of fee
Permanent residence card
Passport
Life in the UK test
English Language Certificate
Is there anything else I am missing?
In the AN form, the supporting document part section 3 says Evidence of lawful residence for 5 years such as letters from employers - I am confused whether I should attach these or as I fall into section 4 EEA I should fulfill only section 4 requirements and ignore section 3.
Please clarify. Many thanks again.

Check you were physically present in UK 5 years before date of your application.
Well worth applying via NCS (via local council too)
- an extra layer of sanity-checking & you keep your original docs; (all for a reasonable fee).

2.4 – 2.6 If you are a national of a member state of the EEA, or the family member of an EEA national, and do
not have indefinite leave to remain in the United Kingdom you should complete these sections. Please refer to section 6 of the Booklet AN for further guidance. If you have PR you don't need to fill it up I guess so.

PR is not ILR. If you don't have ILR it would be appropriate to complete the section.

I think I will prepare two copies of the page - one filled in and other left blank - hope the NCS officer will be knowledgeable and will advise.

That's one way of looking at it.
PR & ILR are both forms of settled status; under EU rules people get their settled status via PR instead of via ILR.
Don't forget form AN is multi-purpose & is used by all applicants (whether they got their settled status under immigration regulations or under EU rules).
Even if the questions don't seem to apply or ask about different periods of time (compared to your circumstances), you will submit your PR card which shows you have acquired PR anyway.
