Note the application form is generic & attempts to cover all people's circumstances.
Just work through the questions as they apply to you, cross-checking with the guide and other notes.
Suggest you complete a rough copy first (on a copy of the form); when you are happy it's complete & accurate then make a good copy.
Keep a copy for your reference before you go to NCS. They will copy your documents to send off to HO but usually they will not make a copy of the application form for your records.

If you have an official confirmation letter showing approval from 19 Nov then use that.
(Include letter)
Make sure you were physically present in UK on that date 3 or 5 years earlier - see guidance on this point
2) Question 1.22 - I have PBS print out and master degree certificate , confused which option should I select for this in the form ?
You need to work though the options step by step according to your circumstances & dates
3) Question 1.27 - My immigration history is Workpermit -> Tier 1 -> ILR , should I give my parents details ? or leave it blank ?
Yes - include details
4) Question 1.52 - Address of tax office - what should I wite here ?
Your local tax office if you have had dealings with them (- look on payslips or P60 forms etc).
5) Question 6.2 - Should I write BRP number reference here ? starting witj 2 alphabets and 7 digits ?
Yes
6) Question 6.5 - BRP Place of issue , should I give the place I did premium service ?
UK if in UK; no harm in adding town if you know it.
7) Question 6.6 - Nationality ? - should I give my current Nationality ? (seems bit confusing ,sorry if its obvious answer)
Yes
9) Employment - Hope I can give Month and Year for the dates ?
I believe so - it's to support evidence of compliance with UK laws & to support your 'good character'.
10) Payment slip is
https://www.gov.uk/government/uploads/s ... t_2015.pdf (LAST Page) ?
Yes
11) Question 7.5 - I am applying for myself , should I still give my spouse passport ? or should I not tick this box ?
If applying on basis of marriage include copy of spouse passport otherwise leave it out & don't tick box.
Don't forget NCS will also sanity-check your application & pick up any glaring errors & omissions - that's why you are using them.

Thank you a ton noajthan. Really appreciate you taking time to reply for my email.
- I did go through Guide and Booklet. IMO these really does not OR cant cover all situations. As in the form it states to select combinations for EG: Language , I have Master degree from India, which is listed in PBS. And in Form I think should select
"I have obtained an academic qualification deemed by UK NARIC to meet the recognised standard of a Bachelor’s or Master’s degree or PhD in the United Kingdom and:
i. UK NARIC has confirmed that the qualification was taught or researched in English ? Still not sure this fits my sitatuion , but will check with NCS as well.As you said I have almost done the Draft form , will do the main form today.
- Our Forums link "read before post" is certainly useful. But it would be great if it can be revisted, as it was written Feb 2012. Most of them would have changed and lot to add as well.
- As I do not have a official letter for ILR approval letter , will go with BRP date.
- Lastly , my query #12 , can you please clarify on that as well please.

For KOLL, my wife submitted a B1 ESOL certificate some am not sure on the intricacies of the degree route.
For #12, any self employment will go in the employment history section.
- the notes to the form ask for supporting docs in the shape of a HMRC tax office
self assessment statement of account.
If you don't have that (or can't get that in time) then yes, anything relevant from an accountant or any related supporting letter (or statement or accounts or payslips/P60s etc) will surely help.
NCS should clarify if its not adequate & the HO will always write to you and ask if they need more information.
My approach has always been to answer the questions as fully as possible; back them up with as much relevant evidence in order to eliminate doubts and questions from HO side (& to reduce their wriggle-room).

This section of form is new to me, (we used previous version, earlier this year).
HO are clearly looking for supporting evidence of 'good character' & a general compliance with UK rules & regulations, including paying taxes.
So if you've only paid tax by PAYE (even when you were self-employed) then perhaps an annual P60 tax certificate will cover it.
For employees HO can easily crosscheck with HMRC so for PAYE it may be additional evidence is not needed.
I'd take recent P60s with me to NCS & see if NCS want to include them in the application.

Sure . I am taking Selft employment P60s and a letter from my accountant stating I am using PAYE and have not arrears to HMRC. Will check with NCS which ones they recommend. Before that what you guys recommend ? Both or anyone ?
Thank you.

If you have more documents than you need it will be all good - NCS can pick & choose & advise what is required from what you are taking with you.
Or they may say for someone in PAYE category nothing has to be submitted.
Good luck.

Hi noajthan,
Update on the NCS happenings. Went there 15 mins early and was asked to wait. NCS guy came and wanted to see my filled docs to check the date and he was complaining from morning most of the applicants came with old form and he had to make him fill the new one. He was happy to see my new form filled. So asked me to wait . After waiting for another 25 mins our turn came.
Now the important bit on docs ,
1) He took my P60's for years I worked as self employed (director for Ltd company) and also the letter from my accountant.
2) He did not bother putting in Head Office reference AND Tax Office references , as we could not find any on my P60 or Payslip. He said Home office can find it them selves. Also there was 1 month gap 4 years back in employment as I needed time to setup my LTD company. Mentioned about it to him , he said it should be fine. Not sure these will delay my process.
3) For language he asked to select "i. UK NARIC has confirmed that the qualification was taught or researched in English;" Page 7 for PBS Print out + Degree certificate route.
All in all He was confident that my case was straight forward

Rajuk,
It's always nerveracking; they (HO & NCS) are not really there to catch us out but just to apply their rules (to the letter).
Sounds like you were well-prepared and so all went well.
You were able to provide the information at an acceptable level and in the format they required.
Good luck.

As per my previous clarifications I got , unless U get a same option in PBS List for selecting your degree , its a risk. Better to get NARIC comparison. In fact I did not apply for my spouse just for this reason. And planning to get NARIC comparison.
Unless If you have used PBS calculator before for applying Tier 1 , and got points then seniors can answer.

Seniors - Luckily I do have a printout from the time I applied my ILR with tier-1 option and bachelors degree. Should that be fine ? As I have my appointment on Monday and now I am worrying ? Thanks

Are you proposing not to submit page 24? You can't omit a page. Further Information must be given on the form provided.

No its not that mean. My mean is that fill & submitted the all whole form and its all pages but adding a computer typed covering letter instead unclear hand writing on page 24. I know its optional but whether ncs staff will let to attach that covering letter and will they forward it??

You can only try. NCS staff should know whether it will be acceptable.

PDF-XChange Editor (free version) or similar products on the form.
