AN form few questions.. please help!

Hello,
I would like to ask for some help with my application from the pros here, some points are just not clear
I am from an A8 country.
1. Name at birth.
My name changed 20 years ago in every possible document, including my birth certificate. If I include the previous name do I have to attach some sort of evidence of change? Is it ok to leave it out "name at birth" as my birth certificate shows the new name together with all other documents? Just trying to save some hassle(?)
2. Do I have to provide proof of all previous addresses with the application? I could not find it saying it anywhere.
3. I have completed a PhD in UK in English (it can be a proof of language), can I just attach a printed copy instead of the original? I have the original in another country, complicated to obtain..
4. Do I have to include all absences from the UK during the past 5-6 years, e.g. trip in France which possibly has no records at all? (No mark in passport). What happens if I forget some... ?
5. Do I have to attach proof of work/studies? In what form?
6. Is comprehensive sickness insurance needed if I was a student in the first 4 years of my stay? Not sure if I ever registered for such.. ?
