Form AN -Questions 6.7 and 6.13

Dear All,
I am in the process of filling out Form AN, and would be most grateful if you could clarify a few queries please.
1. I am hoping to submit the application through the NCS, but question 6.7. asks whether the BRP is enclosed. Does that mean I have to send the original BRP? (I am unable to do that as I am hoping to travel overseas in Dec).
If the NCS check alone is OK, can I tick the box to say ‘yes’?
2. Question 6.13 asks the date the fingerprints were taken which I cannot remember. Would that be a problem? I have had 2 BRPs - the current one in 2013 for ILR (with 10 year rule, and application submitted by post) and the previous one on Spouse dependent visa in 2011 which was by same day service at Croydon. From what I remember, I had to submit biometric data for the second one (ILR) as well, at the post office, but cannot remember any details at all. What should I do?
(I do have the letter including the date/reference number of the first BRP which was returned to the home office when receiving the second, and ofcourse the second BRP which is valid currently )
