My wife is applying for naturalisation using the AN form. We got married in 2012 but she has not changed her name since marriage.
For the British passport she wants to have my name as the middle name and my surname. Does she need to fill out a deed poll or is a marriage certificate enough evidence? All her documents including current passport and bank details have her original name before marriage.

She will need to do the naturalisation and passport process in her maiden name as that is what her current official documentation lists her as. Once she has the passport, she can do a deed poll and then change her name.
