Seat Belt Ticket - Where in the Form AN to declare it?

Where exactly do we declare a Fixed Penalty Notice (such as speeding or seatbelt tickets) in the Form AN. Is it Question 3.1 (Have you been convicted of any criminal offence in the UK or any other country?) or Question 3.5 (Have you received any cautions (simple or conditional), warnings or reprimands in the UK or any other country?).

NCS appointment: Alone or with Family Members?

Hi
This coming Monday afternoon we have an NCS appointment. The applications are for me, my wife and son. I was wondering, though, if I could submit the applications myself. Do my wife and son have to be present at the time of application?

I would assume they would need to be there too.
