BC Application, confused re ILR form previously

Dear Experts,
I applied for ILR along with 3 dependents under tier 2 in july 2014.
Got ILR the same day.
I remember i filled set O form.
What i don't remember wether i have filled separate form for my wife and kids?
I want to know wether we were suppose to fill separate form for dependant in july 2014 or it was only one form set(O).
Please help

Does it matter?
One form together was acceptable.
Apply to settle in the UK wrote:Family members (dependants)
Your partner and children under 18 can apply on the same form as you if they’re eligible.

my wife (dependant) started working in april 2014 as a self-employed. I applied for ilr along with my wife and two kids in end july 2014 and got the same day.
To what we remember we havent declared that she is working as self employed.
now for citizenship we have to declare that.
I dont want my wife application to b refused on deception.
so wanted to clear
i remember that i filled Set(O) 07/2014 form but was not able to figure out wether i filled only one form or i filled separate form for them.
I applied under tier 2 general after 5 years in uk.
I saw form (M) got confused with it

It does not matter. Complete form AN truthfully. She applied as your dependent on Set(O) form.
