Nationality application questions

Hello Experts,
I am applying for the UK nationality in few days time and have some basic questions. Hope you can help with them. So here is my case:
Myself, my wife and daughter got ILR on July 15, 2015 on the basis of Tier 1 general visa(myself being the main applicant). I also have a son who got UK nationality later on after we got ILR status. So my questions below are about our(myself, wife and daughter’s) nationality applications (please note that my daughter was born abroad when we did not have ILR and is almost 8 years old):
i) What is the earliest we can apply, is it July 15 or July16 2016?
ii) How much is the qualifying period in our case, is it 3 or 5 years?
iii) Which application forms to use ( I guess its form AN )?
iv) Can I apply for my daughter at the same time as myself and my wife?
