Paying home office fee (NCS Appointment)

I have booked NCS appointment for Family of three. It is a JCAP appointment. Please guide if I need to fill three payment sheets i.e a separate payment sheet for each family member or I can pay the whole fee using one payment sheet?

Just looked at my copy of the payment form - at the top it asks for applicant name, DOB, place and country of birth. So one payment form for each application. They are processed separately anyway. OK to use same card of course for each form.
