If he's a professional and on the list then all good;
details to be completed as per guidance.

Copy form for your records, NCS usually won't.

Is that certificate a SELT?

Can't think of anything else to add.

Take a copy of your application form for your records retention - NCS only copy the supporting docs.

Keep for reference in case of any dispute or if you are refused and need to fight your corner.

Should be a standard practice for any immigration matter (and other life events too). Keep all such documents - for ever (just as HO does).

I came few times to see my wife in uk in 2004 and 2005 nd 2006 and each time I only had staid a month or less with visit visa and in 2010 I came with entry clearance as spouse of EEA and lived since

First sounds like 2004.
HO will (probably) have own records on you to crosscheck.

First question I got asked at my appointment!
In your case 14-10-2011 is correct.

Hello everyone
Just finished my Ncs appointment
The lady looked like she hasn't got clue of the Application
She only copied
My two passport to cover period residence
Copy of my English certificate
Original life in uk
Copy of my permanent card
Copy of SAR page which confirm my acquired PR date
Checked my application soo quickly she just read it
That's all ,
I declared the allegation to be safe side .
I feel dodgy feeling toward my naturalisation
Hope all go good
