Certificate of Entitlement for baby

I am preparing to apply the Certificate of Entitlement/Right of Abode for our baby born for Settled Father.

In the ROA Form,
If someone is representing you, for example an agent or solicitor, please tell us their:
Do I need to fill my name to say that I am representing for our baby.


Also in the bottom of the application it is asking the applicants signature not sure whose sign should be there.

Experts advice would be more helpful.

No, representing you means a solicitor or agent.

You sign on behalf of the child.

One more doubt in the form
Section 2: Your residence in the United Kingdom
2.1 Please state when and where you entered the United Kingdom
As the baby born in UK not whether to enter the DOB and place of birth or leave it blank.

furthernationalityenquiries@homeoffice.gsi.gov.uk they confirmed that section can be left blank.

Last questions on this topic,

For the proof of the documents babies born in UK after 1983
• Applicant’s full birth certificate showing parents’ details;
• Evidence of either parent’s British citizenship or settled status at time of applicant’s birth, e.g. a passport describing the relevant parent as a British citizen or indicating that they then had indefinite leave to remain; and
• Parents’ marriage certificate (if claiming through father and applicant born before 1 July 2006)
For the second point, sending the original BRP is sufficient to proove the eligibility with copy of passport or should I need send the original passport as well
