Disclosing the fixed penalty issued by council

Hello, apologies for the cross posting, but I have only one fixed penalty issued by council for illegal litter disposal (my flatmates left the black bag with the litter from the house in front of the house) 
I paid the penalty immediately.
I wonder where to disclose it in the form AN (on the page 17 or on the page 22)?
Also, shall I submit the evidence of fixed penalty payment?
