I have an appointment booked on next month for BC, i am self employed since Aug 2015, i am working as private hire driver. My questions are about in the form which are

1.45 What is your occupation ?

1.47 Name of employer or business ?

1.48 Name of employer or business ?

1.49 Address of tax of ce or tax of ce reference ?

Only you will know these question answers if you are self employed, assuming you are self employed through a limited company.
