Hi,

I'm filling up the Form AN for the naturalisation application.

Section 1.5 - State your name as it appears on your passport.
Surname/family name
Other names

Section 1.6 - If the name stated above is not the name you use for all purposes, state
The surname/family name that you use
Other names used


On the guide, it says

1.5 Enter your surname or family name as you want it to appear on your certificate.
1.6 Enter your other names as you want them to appear on your certificate. For example if your name is Taher Mohamed Hashim Al Hassan and you are known as Mr Al Hassan then put Al Hassan in section 1.5 and Taher Mohamed Hashim in section 1.6.


I filled up my surname in Section 1.5, but I'm not sure now where I should put my first name.
Section 1.5 - Other names?

It's definitely not easy.

Other names - this is the place where you put all your other names except for surname so first name would go here and if you have second or third name too (like people from Spain) here is the right place to do it.

So means Section 1.6 is mainly for people as you described with second / third names?
However, where does middle name sits?

You are missing what the member has said.

1.5 is your SURNAME, i.e. Smith

1.6 is ALL your other names, John Andrew etc.

When the middle name is the paternal name (not a second name) and not generally in use in any other documents, how do I fill in the other fields related to name?
I'd rather not create more confusion.

You simple complete the names as they are in the passport. Don't overthink this.
