applying for naturalization

hi friends and respectable seniors!
i am due to apply for naturalization and confused about few Q on Form AN:
A)page 14 points 3.3 do you have any civil judgment or any civil penalty?(as i have three penality points on my driving licence while i was on phone which was happend in april,2014 and i did not mentioned it on my ILR application,however,what should i do now?just tick on NO OR YES option?
B)what kind of supporting documents i need to submit with my application as i am director of a limited company and an employee of the same company?
