questions from online form

Hey guys
am finalising everything on my form.. i am however stuck on few sections
2.1 : Enter the date you first arrived in uk (guidance says with the view of staying on long term basis in uk)
should i enter the date when i came first time as a student or the one before i qualified for tier 1

Convictions and other penalties section
The section online says
At any time have you ever had any of the following,
in the UK or in another country?
The PCN i have from local council will it go here ? altho the guidance talks abt FPN and not PCN ?

Have you made any previous UK immigration Yes
applications in the UK or abroad?
I came here as a student so in the process ad being in uk in 10 years i have applied for
student few times
tier 1
tier 1 extention
ILR
which ones need to go there as i dont have exact details of previous applications and nor does it ask same in paper form ?
