Form AN Help Please

Hi all,
So I have my NCS Appointment next week and I just wanted to double check everything and I noticed that they just released a new form
but I have my referee information filled out on the April release of the form... My question is do I have to get them filled out again, which could prove problematic, or can i use those two pages from the april form if they didnt make any changes to them in the june form? (if that makes sense)
