Parents Consent form for MN1

Hi,
I am applying for naturalisation for myself and my son through the NDRS+JCAP. I have completed my online form including for my son.I have downloaded the referee forms and the biometrics forms. I have downloaded the checklist which mentions a consent form from parents. However it didn't give me a format for the consent form at the end of the application. I understand that I can take a print of Page 27,28 from the paper MN1 application form and submit the same at my appointment. Is this right? Or is there a separate form somewhere on the home office website that i haven't been able to locate. Please advise
