Page1, Section 1.1, Reference numbers?

Hi, I am in process of finishing my AN paper form (I am applying as a EU Citizen) and have a question:
Page 1, Question 1.1 “Give any reference numbers used in your immigration applications”
Do I put the reference number for my Permanent Residence application?

Quote all ref. numbers you got. If that's the only one quote it.

Page 1, Question 1.3 Date you were given indefinite leave to enter/remain in the UK.
Do I put the date here when I was deemed a permanent resident?

Hi, I am in process of finishing my AN paper form (I am applying as a EU Citizen) and have some questions to ask.
Page 10, Question 1.46 can I tick more than 1 boxe as I am all three, employee, director and self employed.

Multiple AN Form questions help needed please

Hi, I am in process of finishing my AN paper form (I am applying as a EU Citizen) and have some questions to ask:
-Page 1, Question 1.3 Date you were given indefinite leave …
Do I put the date here when I was deemed permanent residency?
-Page 10, Question 1.46 can I tick more than 1 boxes as I am all three, employee, director and self employed.
-I am submitting a paper form and don’t understand where I need to send my payment – slip.
-Page 31, In the Supporting Documents, Section 10, Evidence of National Insurance contributions covering the relevant period... do I need to send in payslips and letters from employers? or are P60's enough?
My relevant period is the last 10 years and I've been mostly paid weekly and only have the last 3-5 years of payslips.

Multiple Occupancy, question 1.46

Hello Everybody,
I have multiple occupancy as I am all three an employee, a director and also self employed. Does anybody know what shall I tick in question 1.46 in the AN form? Shall I tick all three or choose 1?
Any help would be much appreiated. Many thanks.
