Maiden name on EU ID card - include or not?

I am taking my Life in the UK test in a few days and now am worried about them accepting my ID.
I registered with my German ID card.
It shows my name as:
"XXXXX
GEB. YYYYY"
where YYYYY is my maiden name. My actual surname that I use is only "XXXXX" so that is what I enter in the field "surname" when I complete any forms.
This is also how I applied for my Permanent Residence and it was no problem.
Sorry this might seem a bit confusing...To clarify, here is an example:

When I took the English Language Test, the guy at the Test Centre told me that I would have problems with the Home Office because the name on the form does not match the name on the ID. I think he put a comment in the file, but he still let me take the test.
I don't know what to do. Travelling to take the test is a major hassle and I don't want to be turned away. Obviously, all my residence documents, tax documents, etc. also only show my name as "XXXXX", not "XXXXX geb. YYYYY"
The really big question is now also what to put in my AN form?? I certainly don't want to have a British passport that says "XXXXX GEB. YYYYY"! I would think if it was fine for my PR application (where I submitted my passport with the same information in it), it should be fine for Citizenship?
