Working history if self employed in the past for Naturlisation

Dear Team,
Just a quick question , if you have been self employed in the past, do you have to write it down the duration where it ask for 10 year employment history ?

Also another question please , today I received my last 5 year employment history , I started work in 09/13 and ended 11/15 with one company , they used to give us payslips whole that time , but there is a gap between the employment history
For 5 months starter with different name (agencies)after 5 months
My question is what information should I put in the application form ?

For 5 months different agency starting with different name after 5 months (no paper work given to sign that if agency was changed also
My question is what information should I put in the application form ?

You don't need to provide any documentation for your employment history. Just add all the employment details. You should mention self-employed details as well.

Hi , thanks a lot for your answer ,
Should I enter details what is appearing in my HMRC employment history? Or should I put the details on which I actually worked ,on HMRC record it’s 4 months Gap( agency starting with another name at same work place )
Also Please if any one experienced this share their experience

You should put details you actually worked and you can write a cover letter explaining your work history. If caseworker needs further explanation s/he will contact with you through the post.
