British passport application form query

Hi,
I have 3 queries.
I obtained ILR(10 years route) in May 2019
Baby born in June 2019
I am applying for my baby's first British passport now.
On the section 4: Parent's details which says
Nationality and citizenship at the time of applicant's birth
What information do i need to put in there?
2. Regarding Countersignature. Is that part also needs to be filled in?
3. While submitting application, do i need to send my passport as well or just the BRP is enough?

You put your nationality that you hold. It isn't a trick question.
2. Regarding Countersignature. Is that part also needs to be filled in?
Yes, you need a countersignatory.
3. While submitting application, do i need to send my passport as well or just the BRP is enough?
Passport and BRP.
You can apply using the one week fast track service.

Hi again,
The guidelines says countersignatory must be working in recognised professions.
But the person i know does not fall under those professions mentioned on the guidelines.
Any ideas?

Then find someone else that does. They must also be British.

Hi,
Just a quick question.
On the back of a photograph, is it ok if I write myself this sentence "'I certify that this is a true likeness of...." and then take relevant person's signature?

No. The referee must write it as they are certifying it is a true likeness.

No. The referee must write it as they are certifying it is a true likeness.
