how to get parent consent form for applying kids british passport ?

Hello,
I am looking to apply online for my kids first british passport. When i applied they are asking for some additional documents including Parent Consent form. Any Idea how to get this form or shall we just type a letter and mention all responsibilities as a parent for the child on the letter ?
