Hi All, I am soon going to apply for my naturalisation and have one question about address history.
I live in London and had moved to Birmingham for 4 weeks for some sort of job opportunity so had to change address with DVLA on my driving licence, now am back to London to my same address.

Do I mention this in address history or not as it was only 4 weeks and afterwords i moved to the same previous address. thanks in advance

I mean to inform home office about the address change, is that necessary ?

(1) writing surname in SURNAME box and leave GIVEN NAME empty (not sure if it will allow me to)
(2) write her surname in both Boxes

also in employment section do i only write details of being employed or also self-employed coz during my 10 yrs employment i had also remained on 1 yr self employment.

on the referee form it says to write the D.O.B and name of applicant the glue it on the form, but if its glued to the form the name and DOB cant be seen when uploading the documents. whats the correct way of doing this & also does the referee need to sign the photo as well or just name and DOB

Ignore that bit. You dont have to do that as with the new system the referee forms are scanned and uploaded. The referee only signs the referee form after you input all his/her details in the online form.
