Which FORM and DOCUMENTS require for ILR ON 10 years basis

Hi All,

I really need this material information as I am going to apply for my ILR on the basis of 10 years.
Currently I am on tier-1 (G) expiring on mar-2015 but I am meeting 10 years criteria on 16-Feb 2014.
Would anyone kindly provide the answers for the following g questions. I really appreciate the time you would consume here.

Which form do I need to apply for ILR ON 10-years basis & currently I am on tier-1 g
Which documents would I be needed along with the application form.
Can I apply on the same time for my Mrs & 2 kids or I would have to apply later for them. I must mention here that my Mrs came in Aug-2009 in UK and she left the country to go back
home in April-2013 and she coming back this mar now. Both of
the kids borne here.
If not now when my family can apply for ILR and what are the requirements for that.

Sorry for long writing but I thought its necessary to provide the information for the right ans.
