ILR for wife(seperately)- few queries

Hi, I will be applying for my wife's ILR soon based on Tier 1 dependant route. Please can you answer my queries below in the SET(O) form.

I came to UK on Tier 2 in April 2009
spouse came as Tier 2 dep in Jan 2011
I was granted Tier 1 in May 2011(wife stayed on Tier 2 dependant)
Wife went back in July 2011
Wife came back in Jan 2012 as Tier 1 dep
Myself & wife's Tier 1 extension in March 2013
I was granted ILR in April 2014

In section 6 of the form, the date on which the application is based is Jan 2012(when she first came as Tier 1 dep)?

In section 7, how long have you lived in the UK counts from Jan 2011, plus she had 6 months absence from Jul 2011 to Jan 2012 as we had a baby outside the UK. Should I mention this?

In section 3, the category is PBS dependant?

It's not that important, yes PBS dep,just put the earliest date.
