Need your Kind Help Please..

Dear Friends
I want to apply for my ILR on basis of 10 years long residence. I have my following documents
1.Application form 04/12
2.Passports
3.Photos
4.6 months Salary slips
5.6 months Bank statements
6.Life in UK test
7.All student visa extension letter from UKBA
8.All absence list
9.Bank draft £991
My questions
A.Is there any thing missing in my document?
B.Do I need to send also photocopies of my all documents (Beside all ORIGINALS) including application form or only Original documents?
C.Is covering letter is must with application?
I would be highly grateful to you for your kind help.
