ILR application Question for source of earning

hi guys,
In section 9 - A5 of the set o form. its asking source of earning.
I get dividends from my ltd company.
so do i need to put company details only or clients details in this section ?
second its asking Country of
earnings
(this is the
country where
the work was
physically carried out)
i went outside for project but paid in UK so do i need to put outside country name or UK?
please advise
