I entered UK on student visa on Sept 2004 renewed it till Oct 2006. Later went back to india and i didn't renew my visa..and it got expired.. Later i applied for HSMP on Oct 2007 a fresh visa and been in UK for almost 5yrs...I am in due for ILR according to the 5yrs using my current HSMP visa from Oct 2007, due in Sept 2012...In the application form do i have to show my intial entry from Sept 2004 or can i show my 5 yrs document from Oct 2007.. Can senior members guide through it...

Question 6.1, on page 12 of the SET(O) form, you need to provide the date you first entered the UK on HSMP visa. They are not asking you to give the date when you first came on student visa.

In Question 6.3, you need to give the date when you last entered the United Kingdom.

last entry to UK should be any holiday return recently right...as Jan 2011 return from holiday in india...right?

The last entry will be the date you last entered the UK before you apply for ILR. So unless you are planning to take any other trips outside of the UK before you apply for ILR in September then it will be the last date you entered the UK; if this was January 2011 then that return date is what you need to state.

Yes, your last entry to UK should be any holiday return recently.
