28 days + preferred name Queries

Dear all,
I have booked my appointment for ILR next month.
Could anyone please clarify whether I am eligible to apply 28 days before the 5 years of current residency based on the following information.
1. I came to UK initially on work permit Sept 3rd 2005.Went back to home country after 20 days.
2. Again returned back to UK on the same work permit on July 27th 2007 (as the initial work permit was valid till Dec 2007).
3. Extended VISA in UK (in Dec 2007 till Dec 2009).
4. Due to personal reasons had to take back the subsequent Visa extension application in Dec 2009, but applied for entry clearance (TIER2 ICT) from home country and came back to UK in Jan 2010. Total 28 days out of country during this entry clearance and also Visa discontinuity for 9 days.
5. My total out of country days from July 27th 2007 to till date are 50days.
Am I eligible for my ILR application on July 2nd 2012? Could anybody please clarify?

I have really gained a lot from this forum and looking forward to get some help for my post.

My main concern is about 9 days Visa break (point 4 in the post). Will this be a problem? Can I submit the proof that I am active in UK by showing the bank statements for Salary, utility bills etc. during that period?

I am not able to find any similar post in this forum. If this is already answered before, could anyone please point me to that post.

That was very helpful and gives some hope.
I understand that the gap in visa need to be well justified. I have a copy of letter from UKBA that I applied for Visa extension in country but taken back and also a letter addressed to UKBA from employer requesting the passport back because of travel urgency.

I am hoping to submit the above documents during my appointment next week.
