Date of first entry

I'm a bit of a tricky case and would like help on how to fill in section 6 - immigration history on the form.
I got working holiday in 2004 and was in and out of the UK a lot while on this visa, and partly lived outside uk.
I applied for WP1 in 2005 and was in UK at the time I applied for and received WP1
I got WP2 in 2008 and T1 in 2009.
I am just now applying for ILR.
What is my first date of entering uk for Question 6.1?
For 6.2 do I fill in all entrance/exits or just those in the first 5 years? The older dates are difficult for me to confirm, and I'm worried about that.
