PEO APPOINTMENT coming up....

Hi guys,
Have got a PEO APPOINTMENT at Solihull coming up. My route was HSMP 2 YRS followed by Tier 1 general. Applying with my wife (married and staying for 10 yrs).
Do I need 2 set of SET O forms or just 1?
I have HMRC tax statement for last 3 yrs but only 1 p60 (current) is that okay?
I have 12 months payslip and bank statement but don't have all of 5 yrs (have about 60 percent) is it okay?

Are you serlf employed/ Contractor/ salaried employment

Sorry I am salaried. I am a doctor working for NHS. I have got all documents for last 12 months but quite a few docs are missing for last 2 yrs or 5yrs if they ask for it.

You need just 1 form. Select Yourself and 1 dependent - Costs around £2066

HMRC Tax statement should be okay. Can you get the HMRC statement for last 5 years ?

Thanks for your help. I have asked for 5 years but may not get them before my PEO appointment. Hopefully they won't ask for it. They had my payslips for up until 2 YRS ago so don't understand why they need full 5 years anyway.

Had my ILR done today, my case was simple...
CW looked at the form, mainly absences ( mine was total abou 120 days), then asked for 12 months payslips, checked with the bank statement. asked for some cohabitation documents in addition to the bank statements in joint name...they wanted some docs from 2012, 2011 and 2010, took my council tax statement.
Did not need any photocopies or P60. Was really starightforward. Paid by CC.
