ILR/Tier1 2nd Extension

Hi,
ILR/Tier1 2nd Extension.
Can anyone please help me.
i have 1st enter UK - 30th Sep 2007
i have received my first Tier 1 - 10th Oct 2008
i was not in Uk from -10th Oct 2009 to 30th July 2010 (Health issue unable to come back UK during
that period
1st extension 29 Sep 2011th (2 year visa)
Now i have visa till 29th sep 2013
Total absent in UK (350 days)
i am not understand the new rules.Can anyone advise me do i need to apply ILR or Tier1 2nd extension.
