Tier 1 ILR application SET(o) with 1 dependant

Hi All,
I am new to this forum. I have a couple of questions regarding my ILR application. Hope some one can help me with this.
My self
I am currently on Tier 1(general) visa and will be applying for ILR next week.
First obtained HSMP on 08/02/2007
Renewed Tier 1 general 18/03/2010 (this is date printed on the visa)
I have been out of the country a few times during the last 5 years. Followings are the days I have spent out side the UK all on fully paid leave.
Period - - - - - - - - - - - - - - - - - No of days spent out side the UK
Depart 12/07/2009 - - - - - - - - - - - 36 (Holidays abroad / annual leave)
Arrival 16/08/2009
Depart 03/04/2010
Arrival 28/04/2010 - - - - - - - - - - - 26 (Holidays abroad / annual leave)
Depart 25/10/2010
Arrival 02/11/2010 - - - - - - - - - - - 9 (Marriage , Annual leave)
Depart 04/05/2012
Arrival 01/07/2012 - - - - - - - - - - - 59 (Holidays abroad / annual leave + banked up leave from last year, all paid)
Total Number of dates 130
My partner
My wife arrived in the UK on 21/12/2010 as a tier 1 dependent.
She hasn’t got many documents to prove that she has been living with me at the same address apart from the NI letter on 01/02/2011 and quite a few documents/pay slips during the two years after the NI letter, with a total of 5 months gaps in between where she has not received a single letter.
Questions:
1) Do I need to provide proof for the days I have been out of the country, I don’t think I have to as I am a tier 1(general) migrant ????
2) Will there be any issues with my wifes ILR dependant application as I do not have documents to prove that she has been living with me during the whole period? [/b]
3) She changed her name and took my family name (changed her passport, visa is still stamped on the old passport with the old name). Will that be an issue? Do I have to send our marriage certificate with the application?
4) Do I have to send the orginal HSMP approval letter with the points I scored?
