Confusion for 5 Years period

I am on HSMP Tier 1 (General) Visa at the moment, I have got my visa from 28th August, 2008 to 11 August 2011 and got an extension from 11 August 2011 to 11 August 2013 .My ILR is due in August 2013 and I am sort of all most 3 weeks to complete my 5 Years so not sure what should I do ?
What should I apply for?

if you entered UK within 90 days of your EC Date, then you can apply for your ILR 28 days before 11-08-2013

I presume the first EC was issued out of country. Could you tell us the date you entered in the UK.

when I got my HSMP visa first time I was in UK only as I applied for it in country.
