I have ILR. My spouse was initially on tier 1 dependant(4.5 years) and is now on Tier 2 General. Can my spouse apply for ILR using set(M) form as my dependant?

No she cannot unless she gets converted as a dependant on your visa she cannot.

Why did she change to Tier 2 (General)? It reset her qualifying period for settlement.
