Section 11 documents

Hi All,
I do require clarity in completing section 11 - Set (o)
Visa Type from date to date
Work permit 02-Jan-08 02-Jan-10
Tier 1 22-May-09 22-May-12
Tier 1 extn 28-May-12 28-May-14
have completed 5 yrs and 2 months
please do let me know which boxes i do need to tick.
My salary after all deduction varies each month , should i take a average and write under section 5 , 5.3
whilst on Work Permit i had availed 25 days annual holidays and traveled overseas, should i get a letter from my employer for this absence.
