I am goin to pay with my wifes card,Do i need to fill my wife's details as the joint account holder on page 51 and does she need to sign that as well.

I would suggest keep that open, you can always ask it at PEO first counter they look at your app form before payment of fee.

As an easy one, you can fill this page along with wify's permission on a letter signed by her. And also carry a blank print out of that page to enclose just in case.
