Which Form to use??

Hi there,
I am applying for Indefinate Leave to Remain on the basis of completing my 10 years in The UK ( Student + PSW + Tier1 + Tier1 Ext ).
Kindly please advice me on which application form to use? is it Set(O) or Set (LR) ??
