Help with SET(O) question Section 6.1

Hello all and many thanks for the replies to my questions so far. I'm in the midst of filling in the SET(O) form. The question asks for the date I first [b]entered[/b] the UK. I am tempted to take a literal interpretation of the question to mean physical entry however I am told that I can use the date my entry clearance was granted, as long as it I arrived within 90 days. Therefore my questions are:
1. Should I enter the entry clearance date or the date I physically entered the UK on the form?
2. If I use the entry clearance date (9 January 2009) can I apply 28 days before which is 13 December 2013?
